Probioma Gut Health - Instructions for use

Step 1: Register the test kit

• Click the "Register your test kit" button at the bottom of this page
• Enter the code on the test tube.
• Enter the "Therapist Code" you received from your therapist.
• Fill in your contact information and create a password to complete registration.

Step 2: Questionnaire

• After registration, answer all the questions that appear on the screen
• Press "Submit" to share your answers with us.

Step 3: Sampling

• Use the included swab to take a sample the size of a lens
• Transfer the sample to the tube and shake for 30 seconds
• Discard the swab and close the tube tightly.
  

Step 4: Return

• Place the tube in the envelope and seal it.
• Return the sample using the return label provided with the test kit.

Step 5: Results

• Open the mobile application and log in with your email address and password.
